confirmed to have COVID-19?

Have you had laboratory exposure while working directly with
specimens known to contain COVID-19?

Have you been in a setting in the last 14 days that has been identified as a risk for acquiring COVID-19, such as on a flight, at a workplace or an event?

If symptomatic: Do you or the person you are inquiring about have a chronic health condition that you are concerned about?

Please review the details of this screening tool and inform if your health status changes.

If you have tested positive for COVID-19, I will request proof of recovery before booking your next appointment.

If a child is under 18 a parent must be present to sign forms; parents of younger children must attend the appointment. Please note that appointments of this nature will be limited to times when the government mandated clinic capacity will not be exceeded. Masks must be worn at all times in the clinic.

New health and safety guidelines mandate social distancing and limit the number of individuals allowed in the clinic at a given time. As a result, children and other family members will not be permitted to accompany you to your appointment. Please prearrange care for your children; if you are unable to do so we will have no choice but to reschedule or cancel your appointment.

During the pandemic, I am waiving fees for cancellations due to illness. If you present any signs or symptoms, please call as soon as possible to cancel your appointment. If you show up to your appointment presenting signs or symptoms or withhold any health information, I reserve the right to cancel your appointment immediately and charge the cancellation fee. This is for the safety of our therapists, clients and families.

Before Entering Clinic

In order to operate under the current health and safety guidelines, there is a limit to the number of people allowed in the clinic at a given time. As a result, our reception area is not available; please wait in your car or across the parking lot and only approach the clinic 5 minutes before your appointment at the earliest.

As part of the health and safety measures to protect ourselves and our clients, and prevent the spread of COVID-19, each client must undergo a screening process. Please be sure to read all of the following information before your appointment. A detailed screening questionnaire will be attached to your confirmation email which must be sent prior to arriving at the clinic – if not, the questionnaire will be conducted before you enter the clinic on the day of your appointment. If you present with any signs or symptoms of illness, your appointment will be cancelled and we will ask that you call Health Links.

As a result of our capacity limit, our receptionist will not be available and we will be preparing our own paperwork and insurance forms for the time being. Without the support of our receptionist we will be restricted from making calls to insurance companies. If you have insurance, please call your company to be sure it is still valid before arriving to your appointment. If your claim fails, you will be asked to pay up front and I will issue a receipt to you.

We will be sanitizing the clinic between each appointment, and after contact with specific areas. The restroom will be available but we ask that you please try to use the restroom at home before coming to your appointment.

During Your Appointment

Upon entering the clinic you will be asked to sanitize your hands immediately. Please wash your hands thoroughly after coughing, sneezing, touching your face or using your cell phone. Some therapists may ask you to wash your hands and arms if you wish to have them massaged during your treatment.

Please shut off or silence your phone while in the clinic and refrain from using it where possible as cell phones can be difficult to keep sanitary. If you must use your phone, please sanitize yours hands again after each use.

All of our therapists are required to wear face masks for your safety, and some may opt to wear gloves during the treatment. Please inform of any allergies you may have prior to your appointment.

It is requested that you wear a mask to your appointment

There is a designated area in the treatment room where you may put your shoes and clothing during the appointment. This area will be sanitized between each appointment.

Payment and scheduling will now take place within the treatment room to maintain physical distancing. If possible please try to use a contact free payment method such as Interac e-transfer or Interac Flash (tap). If you are unable to do so, know that payment consoles will also be sanitized between uses.
