If you have insurance with one of these companies and would like me to direct bill, please provide your insurance numbers at your next appointment. If you would like to book an appointment as a new client, be sure to include your insurance information on the intake form.
Please note that some insurance plans do not allow service providers to bill directly. Direct billing is offered as a courtesy; in the event that any claim fails, it remains the clients responsibility to cover the cost of their appointment and receipts will be issued for reimbursement.
